Medication Forms:

Prescription medication MUST have a signed order from the doctor/nurse practitioner who prescribed the medication.  No medication can be given without that order.  Medication forms are in each building office as well as the nurse’s clinic.

Chronic Condition Forms:

Asthma

• If your child has asthma-especially if they have an inhaler they may  need at any school related activity, please fill out this form and return it to the school nurse.

School Asthma Form

Epilepsy/Seizure Disorder:

• If your child has been diagnosed with epilepsy/a seizure disorder, please fill out this form and return it to the school nurse.

Seizure Action Plan

Serious Allergy Condition:

• If your child has a serious allergy-especially if it requires an Epipen/Auvi Q device or other allergy medication, please fill out this form and return it to the school nurse.
