Immunization Requirements

Ohio Immunization Summary for School Attendance 

Be sure to send in your child’s updated shot records if your child is in kindergarten, 7th or 12th grade OR you have been notified that your child’s shot record is not up to date.  This is a state law, and your child can be excluded from school if the proper paperwork is not submitted.

Immunization records are due within 15 school days after the start of the school year or  within 15 school days of starting school if you are a newly enrolled student.
